Nursing Center Wound Manager

About the Role:
We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Wound Care RN at Chapel Manor in Philadelphia, PA. As a Wound Care RN, you will play a critical role in leading our skin health and wound management program, collaborating with an interdisciplinary team to provide exceptional patient care.
Key Responsibilities:
- Lead and integrate a fully developed Skin Health and Wound management program.
- Collaborate with nursing center management and staff, clinical leadership, Wound Specialists, vendors, and others to effectively communicate with patients and their families regarding the prevention and management of skin and wounds.
- Support central supply as the clinical expert for skin, wound, and incontinent products and provide oversight for durable medical equipment related to the prevention and management of wounds.
- Participate in a bonus program for achieving nursing center skin health and pressure injury prevention goals and objectives.
- Receive reimbursement for wound certification and annual fees.
Requirements:
- Must possess a RN license (MPT/DPT with leadership review/approval).
- Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire.
- Commitment to attend monthly support calls is required.
- Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire.
- Clinical experience in a skilled nursing setting is strongly preferred.
- Discharge planning experience is strongly preferred.
- Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team.
- Experience in team building and demonstrating respect for the interprofessional team in wound management.
- Knowledge of nursing home regulations and survey process is required.
- Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required.
What We Offer:
- Competitive salary range: $87,000 - $90,000 per year.
- Full-time position with a desirable Monday-Friday dayshift schedule (as part of a nursing center team, occasional evening/weekend hours may be required).
