Hospice Music Therapist Job Description

Masonicare Home Health & Hospice is seeking a compassionate and skilled Music Therapist to provide direct services to hospice and palliative care patients and their families in homes and inpatient and outpatient units.

Key Responsibilities:
  1. Assessment and Intervention: Assess patients of all ages to identify needs and provide interventions that meet individual needs, collecting data in collaboration with the patient and family.
  2. Initial Assessment and Planning: Perform the initial assessment of needs during the first visit and document those needs within the appropriate time frame, assessing emotional well-being, physical health, social function, communication abilities, and cognitive skills through musical responses.
  3. Care Planning and Implementation: Plan and implement care for patients of all ages based on needs assessed, ensuring individualized care, identifying and documenting problems, and setting developmentally appropriate goals, time frames, and interventions.
  4. Direct Patient Care: Provide and document direct patient care according to the Care Plan, within set target dates, and according to all departmental policies, designing music sessions for individuals and groups based on client needs.
  5. Evaluation and Follow-Up: Regularly evaluate patient progress, take prompt action when any change in the patient's condition is noted or reported, and terminate care when goals of management are attained for patients of all ages.

Requirements:

  1. Education and Certification: Minimum of a Bachelor's degree; Masters preferred, Graduate of an approved music therapy program; Board Certified as a Music Therapist.
  2. Experience: A minimum of two years experience and work in the area of end of life issues preferred.
  3. Other Requirements: Maintains current Drivers' License and auto insurance, Possess reliable transportation.

  4. Professional Ethics and Standards: Demonstrates a commitment to recognized professional ethics and standards of practice.
  5. Attention to Detail: Demonstrates ability to frequently concentrate on fine detail without interruption.
  6. Memory and Task Management: Demonstrates ability to understand and relate to theories behind several related concepts and remember multiple tasks given over a long period of time.
  7. Independence and Flexibility: Demonstrates ability to work independently by providing own schedules, prioritizing patient visits, and demonstrates flexibility in meeting the needs of patients and families.
  8. Professional Growth and Competency: Demonstrates commitment to professional growth and competency in fulfilling position requirements.
  9. Understanding of Grief and Life-Threatening Situations: Demonstrates an understanding of the differences between adult and children's reaction to grief and life-threatening situations.
  10. Infection Control and Universal Precautions: Demonstrates ability to adhere to universal precautions and infection control policies at all times.
  11. Professional Image: Demonstrates ability to promote the professional image by adhering to the established dress code of the hospital and wearing name badge as evidenced by supervisor's observation.
