Job SummaryThe Healthcare Leader - Patient Care Coordinator serves as the leader of the healthcare team at Novant Health by establishing authentic personalized relationships with patients and their support systems. This role collaborates with physicians and other team members to assess, plan, implement, and evaluate individualized plans of care that promote optimal health or a comforting peaceful death.
Responsibilities- Deliver exceptional patient experiences in every dimension, every time.
- Leverage diversity and inclusion to support quality care.
- Foster a safe patient environment driven by the principles of 'First Do No Harm'.
- Education: High School Diploma or GED required; 4-year Bachelor's Degree preferred.
- Experience: One year of relevant experience preferred.
- Licensure/Certification: Current RN licensure in the appropriate state required.
$60,000 - $90,000 per year.
