Constellation Health Services is seeking a skilled Physical Therapist Assistant to provide high-quality patient care in a home care setting.
Responsibilities:- Provide skilled physical therapy services to patients in their homes, under the supervision of a registered Physical Therapist.
- Initiate and implement physical therapy plans, including preventive and rehabilitative procedures, in accordance with a written Plan of Care established by a physician.
- Teach and supervise patients and families in the home environment, promoting independence and self-care.
- Coordinate physical therapy with other professional services involved in patient care, ensuring comprehensive and coordinated care.
- Supervise home health aides as needed, providing direction and guidance on simple physical therapy exercises and techniques.
- Confer regularly with the registered Physical Therapist, documenting patient progress and modifying treatment plans as needed.
- Initiate, review, and implement PT Care Plans for each patient, based on assessment and prescribed Plan of Care.
- Observe, evaluate, and report on patient physical and emotional status to other team members and the attending physician, as needed.
- Graduate from an accredited physical therapy assistant educational program.
- Current, unencumbered license to practice as a physical therapy assistant in the state of CT.
- Minimum of one year of clinical experience, with previous home care experience preferred.
- Valid driver's license, auto insurance, and reliable transportation.
