Job Summary

We are seeking a highly skilled Registered Nurse to join our Assertive Community Treatment (ACT) team at Prisma Health. As a key member of our team, you will provide direct community-based services to individuals with serious and persistent mental illness, promoting their health and well-being.

Key Responsibilities
  • Provide medication education services to patients, teaching them about medications, side effects, and medication management.
  • Under the direction of the team psychiatrist, manage the administration of medications to patients and ensure integration of medication education into treatment plans.
  • Provide phone support to patients during non-business hours and respond to crisis situations.
  • Collaborate with the team psychiatrist to coordinate medical assessments, make referrals to community physicians, and coordinate psychiatric treatment and medical treatment.
  • Act as a liaison and consult with families and community agencies to maintain coordination of the patient's medical treatment process.
  • Assist patients in developing basic living and social skills essential for independent living, including interpersonal communication, community resource utilization, and relapse prevention.
Requirements
  • Associate's Degree in Nursing (BSN preferred)
  • Two years of psychiatric/mental health nursing experience
  • Licensed to practice as a Registered Nurse in South Carolina
  • Valid driver's license for South Carolina
  • ANCC certification in Psychiatric Mental Health Nursing (preferred)
Working Conditions

This is a non-management position that requires working various shifts, including an on-call rotation of nights and weekends. The ideal candidate will be comfortable delivering community-based services and have a strong commitment to providing high-quality patient care.
