Job Summary

Community Access Inc is seeking a Peer Bridger to join our team in supporting Adult Home Residents in their transition to Supported Housing. The ideal candidate will have a strong understanding of recovery-oriented services and be able to build mentoring and supportive relationships with residents.

Key Responsibilities
  • Develop and implement individualized plans to support residents in their transition to Supported Housing
  • Provide peer-based services to identify and overcome barriers to housing transition
  • Build self-advocacy and systems navigation skills in residents
  • Collaborate with community partners to provide resources and support to residents
  • Participate in regular team meetings to discuss resident progress and challenges
Requirements
  • High school diploma or equivalent required
  • New York Peer Specialist Certification and/or Howie the Harp graduate preferred
  • Minimum of two years of experience in behavioral health setting
  • Recipient/survivor of mental health services required
  • Strong understanding and practice of recovery, wellness, and related services
  • Strong oral and written communication skills
  • Ability to utilize various computer programs, specifically Microsoft Word and Excel
  • Ability to travel and work afternoons, evenings, weekends, and holidays
Preferred Qualifications
  • Bilingual candidates encouraged to apply
  • Experience working in New York City mental health supported housing
