Primary Care Provider

We are seeking an experienced Primary Care Provider to join our team at Ennoble Care, a mobile primary care and palliative care service provider. Our clinicians provide continuum of care for patients with chronic conditions and limited mobility in New York, New Jersey, Maryland, DC, Virginia, and Georgia.
As a Primary Care Provider, you will be responsible for providing high-quality care to homebound and palliative care patients in the Central Ocean County, NJ region. Your duties will include assessing patient health, performing physical examinations, obtaining medical histories, and creating personalized care plans.
You will also be responsible for communicating effectively with office staff and clinical operations, attending quarterly medical staff meetings, and developing impressions, diagnoses, and treatment plans. Additionally, you will foster trust and consult with patients, families, and caregivers during visits, order diagnostic testing as medically necessary, and prescribe medications, supplies, and interventions.
To be successful in this role, you should have strong clinical skills, a positive demeanor, and excellent people skills. You should also be able to confidently problem-solve and operate independently. A valid NP or PA certification in the state of New Jersey is required, along with 1-2 years of primary care experience with geriatric patients.
