We provide compassionate care across various settings, including assisted living, independent living, skilled nursing, and long-term care facilities throughout the United States.
Key Responsibilities
- Perform clinical interventions and episodic care visits, along with necessary follow-up evaluations.
- Manage ongoing treatment for chronic and co-morbid conditions as clinically appropriate.
- Assess and address changes in residents' health status.
- Review and manage medications, orders, and diagnostic tests, responding to clinical issues as needed.
- Collaborate with physicians for comprehensive medical management.
- Conduct Annual Wellness Visits for long-term care residents and perform Annual History and Physicals as required.
- Assist with facility-wide health concerns, ensuring appropriate collaboration.
- Work alongside other healthcare professionals, including therapists and social workers, to coordinate care.
- Maintain open communication with families and participate in family conferences when possible.
- Undertake additional responsibilities as assigned.
- Valid nurse practitioner or physician assistant license in the state of practice and current board certification.
- Master’s Degree in Nursing or Physician Assistant Studies.
- Preferred strong leadership, interpersonal, and problem-solving abilities.
- Excellent written and verbal communication skills are essential.
- Must demonstrate punctuality, organizational skills, and the ability to work effectively within a team.
