Key Responsibilities
  • Perform clinical interventions and episodic care visits, along with necessary follow-up evaluations.
  • Manage ongoing treatment for chronic and co-morbid conditions as clinically appropriate.
  • Assess and address changes in residents' health status.
  • Review and manage medications, orders, and diagnostic tests, responding to clinical issues as needed.
  • Collaborate with physicians for comprehensive medical management.
  • Conduct Annual Wellness Visits for long-term care residents and perform Annual History and Physicals as required.
  • Assist with facility-wide health concerns, ensuring appropriate collaboration.
  • Work alongside other healthcare professionals, including therapists and social workers, to coordinate care.
  • Maintain open communication with families and participate in family conferences when possible.
  • Undertake additional responsibilities as assigned.
Qualifications
  • Valid nurse practitioner or physician assistant license in the state of practice and current board certification.
  • Master’s Degree in Nursing or Physician Assistant Studies.
  • Preferred strong leadership, interpersonal, and problem-solving abilities.
  • Excellent written and verbal communication skills are essential.
  • Must demonstrate punctuality, organizational skills, and the ability to work effectively within a team.
