Case Manager RN

About the Role:
We are seeking a highly skilled and compassionate Case Manager RN to join our team at Mount Sinai Morningside. As a Case Manager RN, you will play a critical role in ensuring the optimal care and outcomes for our patients. You will work collaboratively with our interdisciplinary team to assess, plan, implement, and evaluate patient care plans, with a focus on reducing length of stay and improving patient satisfaction.
Responsibilities:
- Conduct thorough assessments of patients to identify their clinical, psychosocial, and functional needs.
- Develop and implement individualized care plans that address patient needs and goals.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and coordination.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
Requirements:
- Bachelor's degree in Nursing required; Master's degree preferred.
- Minimum 3-5 years of experience as a RN in an acute care setting.
- Current NYS RN nursing license required.
- CCM certification preferred.
