About the Role
We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at BrightSpring Health Services. As a Hospice Care Specialist, you will play a vital role in providing high-quality care to patients and their families in the comfort of their own homes.
Key Responsibilities
- Provide personal care services to patients, including bathing, dressing, and grooming, as assigned.
- Assist patients with daily living activities, such as feeding, toileting, and ambulation, as needed.
- Monitor patients' vital signs, including temperature, pulse, and blood pressure, and report any changes to the Case Manager.
- Assist patients with range of motion exercises and transfers, as directed by the hospice nurse or therapist.
- Provide emotional support and companionship to patients and their families, as needed.
- Document patient care and progress in a timely and accurate manner.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Participate in quality assessment and performance improvement activities to ensure high-quality patient care.
Requirements
- Meet the training requirements of the State, including completion of a minimum of 75 hours of classroom and supervised practical training.
- Have a minimum of 6 months of experience providing personal care services, including experience working with terminally ill patients.
- Be at least 18 years of age and possess a high school diploma or equivalent.
- Have the ability to read and follow written instructions and document care given.
- Understand hospice philosophy and be comfortable providing specialized care to the terminally ill.
- Have the ability to work with little direct supervision and possess strong communication and interpersonal skills.
