Job Title: Primary Care Nurse Practitioner

Reports To: Chief Medical Officer (CMO)

FLSA Status: Exempt

Status: Full-time (hours/week, schedule varies)

Compensation: Commensurate with Experience

Objective:

In collaboration with a physician at the Health Center, the Nurse Practitioner is chiefly responsible for delivering comprehensive primary care services that align with their training. This includes, but is not limited to, assessment, treatment, care coordination, preventive services, and health maintenance for patients of all ages and genders. The Nurse Practitioner is also expected to understand and support the mission, goals, and objectives of the Health Center, a Federally Qualified Health Center Look-Alike, and work cooperatively to achieve these aims. Additional responsibilities may be assigned by the CMO in accordance with funding agency requirements, contractual obligations, and policies established by the Health Center's Board of Directors.

Key Responsibilities:

  • Ensure high-quality care for all patients.
    • Gather health histories and conduct physical assessments.
    • Diagnose and provide counseling on treatment plans for each patient.
    • Prescribe medications in compliance with state laws, Health Center policies, and professional guidelines.
    • Facilitate referrals for patients needing services unavailable at the Health Center.
    • Review incoming reports (e.g., lab results, imaging) and ensure timely follow-up in accordance with Health Center policies.
    • Engage in staff outreach and off-site health initiatives as directed by the CMO.
    • Adhere to the Health Center's Infection Control protocols, utilizing universal precautions and ensuring compliance among support staff.
  • Diagnose and manage a broad spectrum of acute and chronic conditions affecting both pediatric and adult populations, including but not limited to:
    • Dermatological issues
    • Respiratory conditions
    • Neurological concerns
    • Gastrointestinal disorders
    • Musculoskeletal injuries
    • Otolaryngological issues
    • Genitourinary conditions
    • Endocrine disorders
    • Cardiovascular diseases
    • Mental health and behavioral issues
  • Uphold the highest standards of medical ethics, professionalism, and practice at all times.
  • Assist the CMO and Chief Medical Quality Officer (CMQO) in formulating medical policies, quality improvement strategies, and evaluating existing practices.
  • Participate in peer review, quality assurance, and clinical meetings.
  • Support the CMO in designing and implementing educational initiatives for patients and clinical staff.
  • Provide leadership, training, and mentorship to the clinical support team.
  • Recommend enhancements in clinical operations, patient flow, medical record management, billing practices, and appointment scheduling.
  • Meet UDS productivity standards and fulfill minimum requirements.
  • Execute duties as mandated by funding agencies and Health Center management.
  • Become well-versed in federal and state regulations governing Community Health Centers.
  • Understand federal and state healthcare program requirements and assist in their implementation.
Promote the mission statement of the Health Center. Deliver direct patient care and participate in after-hours call rotation. Supervise trainees at the Health Center as assigned by the CMO.

Qualifications:

  • Graduation from an accredited nursing program.
  • Completion of an accredited Nurse Practitioner Program.
  • Current Advanced Practice Registered Nurse (APRN) Licensure.
  • Current Nurse Practitioner certification in relevant specialty, if applicable.
  • Current CNOR/CRNFA certification, if applicable.
  • Current D.E.A. registration.
  • Insurability for malpractice coverage.
  • Current Cardiopulmonary Resuscitation (CPR) certification.
  • Advanced Life Support and/or Pediatric Advanced Life Support certification.

Skills:

  • Extensive experience and commitment to serving diverse and vulnerable populations.
  • Ability to engage with individuals from various backgrounds, regardless of race, color, creed, religion, gender, gender identity, sexual orientation, national origin, disability, or age.

Physical Requirements:

Specific vision abilities required for this position include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. The noise level in the work environment is typically quiet.
