You will coordinate inter-disciplinary care, including nursing, therapy, social work, and home care aide services, to ensure comprehensive care for our patients.

Key Responsibilities:
  • Provide patient-centered care, using critical thinking skills to ensure positive patient outcomes.
  • Coordinate care with other healthcare professionals to ensure seamless transitions and optimal patient care.
  • Make decisions regarding home health care for patients, ensuring the best possible outcomes.
  • Maintain accurate records and communicate effectively with patients, families, and healthcare teams.

Requirements:

Licenses/Certifications:
  • Current licensure by the State Board of Nursing in Illinois or Michigan.
  • American Heart Association HealthCare Provider BLS certification.

Preferred Qualifications:
  • BSN degree.
  • Experience as an RN in a healthcare environment.
  • Electronic Medical Record and Microsoft application basic proficiency.
