Nurse Practitioner

We are seeking a compassionate and skilled Nurse Practitioner to join our team in providing high-quality care to patients with serious illnesses. As a Hospice and Palliative Care Nurse Practitioner, you will work collaboratively with a multidisciplinary team to deliver patient-centered care that addresses the physical, emotional, and spiritual needs of our patients and their families.
Responsibilities- Provide comprehensive care to patients with serious illnesses, including symptom management and palliative care
- Collaborate with the healthcare team to develop and implement individualized care plans
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Participate in ongoing education and professional development to stay current with best practices in hospice and palliative care
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
- 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
- Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
