The Care Manager (CM) plays a vital role in ensuring seamless patient progression through the continuum of care at Christus Health. This position requires collaboration with patients, families, physicians, and multidisciplinary team members to assess patient needs, coordinate care progression, facilitate discharge planning, and advocate for patients.
Key Responsibilities:
- Assess patient needs and develop personalized care plans
- Coordinate with healthcare teams to ensure timely and effective care delivery
- Facilitate discharge planning and transition of care
- Advocate for patients and their families to ensure optimal outcomes
Requirements:
- One of the following education is required:
- Certificate, Associate, or Bachelor's degree in Nursing
- Bachelor's or Master's degree in Social Work
- Experience in the clinical or acute care setting is preferred
- Current licensure in the state of employment is required
- BLS certification is preferred

PRN
