Physical Therapist Job Description

We are seeking a skilled Physical Therapist to join our team at Acts Retirement-Life Communities. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to our residents, including evaluations, treatment planning, and implementation of individualized treatment programs.

Key Responsibilities:
  • Conduct thorough evaluations to assess residents' physical abilities and develop personalized treatment plans
  • Implement and modify treatment plans as needed to ensure optimal outcomes
  • Provide education and training to residents and their families on physical therapy techniques and exercises
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document resident progress and treatment plans in accordance with Acts' policies and procedures
Requirements:
  • Bachelor's degree in Physical Therapy
  • Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered
  • Experience in a skilled nursing and/or outpatient rehab environment is preferred
