Compensation

The estimated salary for this role is $83,550 - $112,000 per year, based on experience and qualifications.

About the Role

We are seeking a highly skilled Community Health Nurse to join our Home Care team. As a key member of our interdisciplinary health team, you will provide comprehensive assessments, develop individualized care plans, and deliver high-quality patient care in the comfort of clients' homes.

Responsibilities
  • Patient Care & Service: Perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care
  • Planning: Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed
  • Communication: Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly
  • Documentation: Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards
Requirements
  • AS: Nursing (Required)
  • RN - Registered Nurse - New York State Education Department (NYSED)
  • BLS - Basic Life Support - American Heart Association (AHA)
