We are seeking a highly motivated and compassionate Peer Navigator to join our team at SCAN. As a Peer Navigator, you will play a critical role in supporting individuals experiencing homelessness in the Garden Grove, CA area.
Key Responsibilities- Build Rapport and Trust: Establish strong relationships with members by sharing your personal experiences and creating a safe and non-judgmental space for them to open up.
- Health Coaching and Navigation: Coach members to develop strategies that minimize risks associated with common health conditions and behaviors, and connect them to local resources and services.
- Outreach and Engagement: Conduct regular outreach to members at the Nav Center and in the community, providing support for their medical needs and advocating on their behalf.
- Accompaniment and Support: Provide accompaniment and support to members for medical, dental, and other appointments, ensuring they receive the care they need.
- Care Management and Coordination: Collaborate with staff at the Navigation Center and the HIA team to ensure seamless care management and coordination for members.
- Documentation and Reporting: Complete all required documentation and reporting in a timely fashion, including entering patient touch points and encounter notes in the electronic health records system.
- Education and Experience: G.E.D./High School diploma required, plus 3+ years of work experience in a related field, or Bachelor's degree preferred with 1-2 years of experience.
- Relevant Experience: Direct work experience, lived experience, training, or volunteer work with patients experiencing homelessness, as well as those with active mental health and substance use conditions.
- Skills and Abilities: Ability to work in a dynamic, outdoors environment, work independently and as part of a team, organize effectively, prioritize multiple tasks, communicate effectively, and maintain reliable transportation.
