Job Description

You will be responsible for providing personalized patient care services to clients in their homes, as prescribed by physicians or authorized by state/program regulations. Your duties will include:

  • Providing high-quality clinical services within the scope of practice and in accordance with infection control standards.
  • Completing comprehensive clinical nursing assessments to ensure patient eligibility and medical necessity for services.
  • Developing and revising individualized plans of care and/or service plans according to federal and/or state program requirements.
  • Collaborating with other healthcare professionals to ensure continuity of patient care.
Requirements

To be successful in this role, you will need:

  • A minimum of one year experience as an RN preferred.
  • Current RN licensure in the state of practice.
  • Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
