About the Role:

We are seeking a skilled and compassionate social worker to join our team. The ideal candidate will have a Master's degree in Social Work and be currently licensed as an LCSW in the state in which they are applying.

Responsibilities:

• Provide neurobehavioral health services to residents in long-term care facilities

• Partner with specialty clinicians to provide holistic, continuity of care for elderly individuals with neurocognitive disorders and other neurologic and/or psychiatric conditions

Benefits:
