Patient Advocate Position

We are seeking a dedicated Patient Advocate to join our team at HexaQuEST Health, Inc. The successful candidate will serve as a liaison between patients, families, and healthcare providers, ensuring seamless communication and support.

Responsibilities:

  • Advocate for patients' needs and preferences
  • Coordinate care planning and implementation
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Provide emotional support and education to patients and families

Additional Details:

This role offers the potential to work at various correctional facilities, with a 60-minute lunch break. Note: The Computer Logon form is required to be completed and uploaded to the state-specific credential line of the candidate profile prior to start.

Estimated Salary Range: $58,000 - $72,000 per year
