Hospice Nurse Practitioner Job Details

We are seeking a qualified Hospice Nurse Practitioner to support our team at Heartland Hospice. The ideal candidate will have a Master’s degree in Nursing and national certification as a Nurse Practitioner. As a Hospice Nurse Practitioner, you will perform Face-to-Face encounters, gathering clinical information to support hospice diagnosis and prognosis, and communicating findings to the hospice physician for certification or recertification.



  • Conducts physical assessments for Face-to-Face encounters within regulatory time frames established by CMS
  • Provides clinical findings, including medical treatment plans, diagnostic tests, lab results, and medication lists, to the physician recertifying patient eligibility
  • Makes recommendations to the IDG based on Face-to-Face assessment findings
  • Notifies Heartland Hospice when unable to complete a Face-to-Face visit according to schedule

Responsibilities

The scope of practice for the hospice NP is limited to tasks necessary to complete hospice Face-to-Face encounter visits.



  • Reports to the Executive Director or Administrator at Heartland Hospice
  • Assigned to a location for Face-to-Face encounters and clinical information gathering
  • Communicates findings to the hospice physician

Requirements

  • Master’s or higher degree in Nursing
  • Graduate-level nurse practitioner education program accreditation by a national accrediting body
  • Current state license as a registered nurse
  • Current state license or certificate as an advanced practice nurse
  • Current national certification as a Nurse Practitioner (ANCC or AANP)
  • Collaborative agreement in place
