About the Role:

As a Registered Nurse in Home Health, you will provide high-quality, patient-centered care to individuals in their homes. Your primary focus will be on delivering comprehensive nursing evaluations, developing personalized care plans, and coordinating with other healthcare professionals to ensure seamless patient care.

Responsibilities:
  • Conduct initial and comprehensive nursing evaluations to determine primary focus of care
  • Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals
  • Coordinate with community resources to ensure continuity of care
  • Provide education and support to patients and their families to promote health and well-being
Requirements:
  • Current RN licensure in the state of Alabama
  • Experience in home health or a related field
