About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at HCA Virginia Healthcare at Home. As a PRN Home Health RN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities

  • Assess patients' physical, psychosocial, and environmental needs, identifying areas for improvement and developing personalized care plans.
  • Collaborate with the interdisciplinary team to establish and achieve patient goals, ensuring seamless care coordination and communication.
  • Provide skilled nursing care, including wound care, medication management, and education, to patients of all ages and backgrounds.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs, ensuring quality and continuity of services.
  • Participate in on-call rotation and emergency call, providing timely and effective support to patients and families.

Requirements

  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Registered Nurse licensure and a minimum of 1 year of experience in home health or a related field.
  • Associate Degree or Registered Nurse Diploma.
