Key Responsibilities

  • Perform home visits and coordinate client care to ensure optimal patient outcomes
  • Oversee the frequency of visits for each episode and consult with physicians as needed
  • Collect accurate OASIS data, maintain current medication profiles, and report lab values to physicians in a timely manner
  • Attend weekly case conferences and monthly case manager meetings to stay informed and share best practices

Requirements

  • Current licensure as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience preferred
  • Demonstrate knowledge and skill in current nursing practice
  • Possess a valid state driver's license and automobile liability insurance
