Sky View Rehabilitation and Health Care, a skilled nursing facility, has an opening for a Physical Therapist. The ideal candidate will assist in planning, organizing, developing, and implementing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines and regulations. The Physical Therapist will work with a team of highly skilled therapists to create an individualized therapy plan that helps residents reach their highest level of functioning in as short an amount of time as possible.
Responsibilities:
- Assist in planning, organizing, developing, and implementing Physical Therapy services.
- Work with a team of highly skilled therapists to create an individualized therapy plan.
- Interpret and/or instruct in physical therapy policies and procedure to personnel, residents, family members, etc. as necessary.
- Participate in discharge planning, development and implementation of resident care plans, resident assessments (MDS), etc. as necessary.
- Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
- Maintain treatment records, resident files, and progress notes as required and maintain an adequate liaison with families and residents.
- Must possess a current license and registration to practice as a Physical Therapist, PT in the State of NY.
- Must possess current malpractice insurance, license in good standing.
- Dedicated to providing extraordinary compassionate care as a Physical Therapist, PT.
- Energetic, enthusiastic, committed, & passionate about quality care.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents.
- Patience, a cheerful disposition, & willingness to assist any residents & co-workers.
- Excellent communication, customer service, & organization skills.
- Trusted to work independently & in a team environment.
- Able to work well under pressure, flexible, & will work harmoniously with all staff.
- Covid-19 vaccinations are encouraged but NOT MANDATED ; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC.
