Position Summary

We are looking for a dedicated Social Worker to enhance our services. This role will focus on care navigation, helping clients feel comfortable and informed as they explore various care options, whether in-home, assisted living, or nursing facilities. Your expertise will be vital in guiding them through their choices and clarifying what each option entails.

Key Responsibilities

  • Engage with families via phone and in-person to provide reassurance and support.
  • Participate in meetings with clients and legal representatives to clarify options and address concerns.
  • Conduct research and coordinate logistics for facility admissions tailored to client needs.
  • Build relationships within the community, connecting with local care facilities and in-home service providers.
  • Establish and maintain professional partnerships with community organizations.
  • Assist families in advocating for their loved ones with care facilities and medical teams.
  • Collaborate with the marketing team to promote our care navigation services.
  • Maintain accurate documentation as part of the social work responsibilities.

Education and Experience

  • Bachelor's degree in social work or a Master's degree with a minimum of two years of relevant experience in a care setting.
  • At least five years of experience in customer service.
  • Current registration or licensure as a Social Worker in the State of Ohio.
  • Proficient in reading, writing, and understanding English.
  • Preference will be given to candidates with experience in elder care.
