POSITION SUMMARY: The Health Care Aide (HCA) is a vital member of the interdisciplinary team. Under the supervision of the Nurse, the HCA assists Residents with daily living activities to meet their personal, social, physical, and recreational needs. All tasks are performed safely and compassionately, adhering to Residents' preferences and Park Place policies and procedures. The goal of care is to foster independence and optimal functioning of the Resident's mind, body, and spirit, treating Residents with dignity and demonstrating a caring attitude. The HCA is committed to ensuring Resident safety and upholding our "culture of safety" as outlined in Park Place Strategic Directions.
KEY DUTIES AND RESPONSIBILITIES:
- Provides care and assistance to Residents according to their personalized care plans, encouraging participation in daily living activities when appropriate.
- Establishes and maintains therapeutic relationships with Residents and their families through respectful communication, ensuring Resident choice and autonomy in care planning.
- Ensures the rights of Residents are upheld and respected, including confidentiality.
- Delivers comprehensive personal care to Residents, ensuring their daily living needs are met in accordance with Park Place policies and procedures.
- Encourages and supports Residents in engaging in social, physical, spiritual, and recreational activities.
- Adheres to the Park Place philosophy of care, recognizing that each individual has the capacity to make decisions about their life and is a partner in the decision-making process.
- Provides personal care to Residents, including assistance with bathing, dressing, skin, dental, and hair care; mobility and transfer needs; toilet assistance; nutritional needs; and overseeing exercise routines.
- Safely lifts, transfers, and positions Residents following individualized assessments to maintain good body alignment and comfort.
- Assists Residents with mobility as needed within the facility.
- Contributes to the development and ongoing adjustments of Residents' individualized care plans, participating in interdisciplinary care plan meetings and advocating for the interests of Residents and their families.
- Administers scheduled medications and, under the direction of the nurse, provides PRN topical medications in accordance with Park Place policies.
- Observes and monitors Residents' conditions, symptoms, and behavioral changes, reporting relevant information to the nurse.
- Documents all pertinent information on the Resident's chart before the end of the shift.
- Assists Residents with meals by serving and collecting trays and feeding designated Residents, maintaining intake and output records as necessary.
- Performs related clerical duties, such as answering the telephone.
- Assists in orienting and sharing information with new employees, students, and other interdisciplinary team members.
- Actively participates in continuous quality improvement and program evaluation activities, educational sessions, team meetings, and committees.
- Maintains a safe environment for Residents, families, and staff by adhering to established safety policies, standards, and procedures, including emergency protocols and infection control guidelines.
- Reports unsafe conditions and malfunctioning equipment immediately.
- Ensures care equipment and supplies are utilized and stored safely and efficiently.
- Maintains a clean, homelike environment for Residents and staff.
- Performs other related duties as assigned.
