Job Title: Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team at Genesis Home Care. As a Registered Nurse, you will have the opportunity to provide high-quality patient care and make a meaningful difference in the lives of our patients.

About the Role:

As a Registered Nurse, you will be responsible for providing comprehensive skilled care to patients in their homes. You will work autonomously, managing your time efficiently and prioritizing visits for high-risk patients. You will also be responsible for administering medication as prescribed by the physician and educating patients and families on disease processes using teach-back methods.

Key Responsibilities:
  • Observe and monitor patient conditions and perform OASIS assessments to develop an individualized care plan and adjust as needs change.
  • Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients.
  • Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Educate the patient and family on the disease processes using teach-back methods to ensure patient and family understanding.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
Requirements:
  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
