Responsibilities:

  • Provide home-based physical therapy services to patients who have lost mobility, strength, and functionality due to an illness or injury.
  • Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans.
  • Conduct assessments, document progress, and modify care plans as needed to ensure optimal patient outcomes.
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility.
  • Educate patients and their families on the plan of care, established goals, exercises, and self-care techniques.
  • Coach family members on safely assisting with exercises and providing support to patients.

Requirements:

  • Graduate of an accredited Physical Therapist program and active PT license in Michigan.
  • Minimum of 2 years of physical therapy experience, preferably in home healthcare.
  • CPR certification.
  • Knowledge of state and federal home health regulations.
  • Good clinical judgment, strong interpersonal skills, dexterity, and compassion.
