Stamford Health, a well-established and award-winning healthcare system, is seeking a skilled Licensed Practical Nurse (LPN) to join our GI Department team. As a valued member of our team, you will play a critical role in providing high-quality patient care and contributing to our mission of delivering exceptional healthcare services.

Key Responsibilities:

  • Coordinate patient care according to established standards of practice and state and federal guidelines.
  • Provide mentorship to non-nursing personnel and serve as a resource to staff and management in problem identification and resolution.
  • Check schedules, organize patient flow, and greet patients, documenting their concerns and questions.
  • Accompany patients to exam/procedure rooms and prepare them for physical exams.
  • Initial patient interviews, reviewing medical profiles, H&P, and documenting in electronic or manual charting as necessary.
  • Obtain and accurately document patient vital signs, weight, and report any abnormalities to physicians.
  • Assist with specimen collection and testing as needed and document pertinent information.
  • Provide minor treatment as directed by physicians and assist with minor procedures.
  • Provide education as indicated after assessing the learning needs of patients and their families.
  • Document all patient-related activities by the end of the shift.
  • Schedule patient testing and pre-certify if insurance requires.
  • Respond to all telephone messages by the end of the shift.
  • End-of-month checks for expired supplies and medication, stocking rooms, and maintaining inventory.
  • Order supplies as needed, clean exam/procedure rooms and equipment after each patient, and prepare rooms for the next patient and physician needs.
  • Notify patients of test results per physician instruction and follow up.
  • Attend scheduled staff meetings, in-service education opportunities, and participate in continuing education programs.

Requirements:

  • Graduate of an accredited school of nursing.
  • Valid license as a Licensed Practical Nurse in CT.
  • Maintain a minimum of 6 contact hours annually.
  • Excellent customer service and communication skills.
  • Knowledge of medical terminology.
  • Minimum of 2 years of previous experience in a similar position.
  • Ability to read, write, and speak the English language.
