Licensed Practical Nurse

Stamford Health, a well-established and award-winning healthcare system, is seeking a skilled Licensed Practical Nurse (LPN) to join our GI Department team. As a valued member of our team, you will play a critical role in providing high-quality patient care and contributing to our mission of delivering exceptional healthcare services.
Key Responsibilities:
- Coordinate patient care according to established standards of practice and state and federal guidelines.
- Provide mentorship to non-nursing personnel and serve as a resource to staff and management in problem identification and resolution.
- Check schedules, organize patient flow, and greet patients, documenting their concerns and questions.
- Accompany patients to exam/procedure rooms and prepare them for physical exams.
- Initial patient interviews, reviewing medical profiles, H&P, and documenting in electronic or manual charting as necessary.
- Obtain and accurately document patient vital signs, weight, and report any abnormalities to physicians.
- Assist with specimen collection and testing as needed and document pertinent information.
- Provide minor treatment as directed by physicians and assist with minor procedures.
- Provide education as indicated after assessing the learning needs of patients and their families.
- Document all patient-related activities by the end of the shift.
- Schedule patient testing and pre-certify if insurance requires.
- Respond to all telephone messages by the end of the shift.
- End-of-month checks for expired supplies and medication, stocking rooms, and maintaining inventory.
- Order supplies as needed, clean exam/procedure rooms and equipment after each patient, and prepare rooms for the next patient and physician needs.
- Notify patients of test results per physician instruction and follow up.
- Attend scheduled staff meetings, in-service education opportunities, and participate in continuing education programs.
Requirements:
- Graduate of an accredited school of nursing.
- Valid license as a Licensed Practical Nurse in CT.
- Maintain a minimum of 6 contact hours annually.
- Excellent customer service and communication skills.
- Knowledge of medical terminology.
- Minimum of 2 years of previous experience in a similar position.
- Ability to read, write, and speak the English language.
