**Key Responsibilities:**

* Provide comprehensive nursing care to patients in their homes, including administering medications, treatments, and therapies
* Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient
* Collaborate with other healthcare professionals to ensure seamless care coordination and communication
* Educate patients and their families on disease management, self-care techniques, and prevention strategies
* Document all patient interactions, assessments, and care provided

**Requirements:**

* Active nursing license in good standing
* Six months of recent experience as a nurse in a clinical care setting
* Home health experience preferred
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
