Job Summary

As a skilled and compassionate Registered Nurse, you will provide high-quality patient-centered care in a dynamic and supportive environment. You will work collaboratively with our team of healthcare professionals to deliver exceptional patient outcomes and promote a culture of excellence.

Key Responsibilities
  • Support the Primary Care Physician in delivering patient care by gathering and documenting relevant clinical data.
  • Participate in daily huddles to identify individual patient needs and coordinate care.
  • Act as a liaison between patients and providers, addressing questions, concerns, and test results.
  • Communicate effectively with the care team to coordinate individualized plans of care.
  • Support the Primary Care Physician during in-office procedures and examinations.
  • Provide outreach and education to patients in need of services, as identified through quality improvement initiatives.
  • Perform duties within the scope of practice to the highest level of licensure.
  • Provide direct support and guidance to other members of the care team as needed.
  • Able to perform assessment data collection on all patients using tools and forms in the Electronic Medical Record.
  • Adequately assess and reassess pain, educating patients and families regarding pain management.
  • Provide age-appropriate care to patients in the community setting.
  • Consistently document procedures completed in the Electronic Medical Record, including medications administered and follow-up assessments.
  • Deliver medication utilizing the five rights and observe patients for desired and untoward effects.
  • Maintain current knowledge of medications and their correct administration.
  • Establish priorities of patient care based on needs and office resources.
  • Demonstrate the ability to assist providers with procedures and perform services requiring technical and manual skill.
Requirements
  • Graduate of an accredited school of nursing or nursing program.
  • Current New York State Registered Nursing registration.
  • BLS Certification.
  • Minimum one year of nursing experience.
  • Basic computer and EMR knowledge.
  • Excellent communication and interpersonal skills.
  • Ability to assimilate information from many different sources.
  • Must be able to communicate in English both verbally and in writing.
