BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Howell, New Jersey.

Job Summary and Responsibilities

We offer a rewarding opportunity for a licensed Physical Therapist to provide high-quality home health care services to adult and geriatric clients in Ocean County. As a Home Health Physical Therapist, you will be an integral part of a multidisciplinary team, working together to provide rehabilitative care and skilled nursing to clients in their homes.

Your primary responsibilities will include:

  • Conducting home visits to clients in designated geographic territories, assessing their physical needs, and developing personalized care plans.
  • Performing diagnostic tests and measurements, such as mobility and range of motion assessments, to identify areas for improvement.
  • Implementing manual therapeutic exercises, gait training, balance, and other interventions to help clients achieve their goals.
  • Collaborating with nursing staff to develop and implement comprehensive care plans, including medication management and wound care.
  • Providing education and instruction to clients and their families on rehabilitative care and activities necessary for independent living.
  • Documenting client progress, interventions, and evaluations in a timely and accurate manner, utilizing electronic medical records.

Requirements and Qualifications

  • A current New Jersey Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
