Resident Care Manager

We are seeking a highly skilled and compassionate Resident Care Manager to join our dynamic healthcare team. As a key member of our team, you will be responsible for ensuring the overall health and wellbeing of our residents.
Responsibilities:- Develop and implement comprehensive care plans for each resident, taking into account their unique healthcare needs and preferences.
- Coordinate and oversee the work of the healthcare team to ensure that all care plans are executed effectively.
- Conduct regular assessments of each resident's health status, making necessary adjustments to their care plans as required.
- Maintain accurate and up-to-date records of each resident's care, including their medical history, treatments, and progress.
- Establish and maintain effective communication with residents, their families, and healthcare team members to ensure that all parties are well-informed about the resident's health status and care.
- Advocate for residents' health and wellbeing, ensuring that their needs and preferences are respected and met.
- Collaborate with other healthcare professionals and departments to ensure the provision of comprehensive, integrated care.
- Comply with all relevant regulations, standards, and policies relating to resident care management.
- Provide leadership and guidance to the healthcare team, fostering a positive and supportive work environment.
- Participate in ongoing training and professional development activities to keep up-to-date with the latest advances in resident care management.
- Bachelor's degree in Nursing, Healthcare Administration, or a related field.
- A minimum of 5 years' experience in resident care management, preferably in a long-term care setting.
- Proven experience in developing and implementing effective care plans.
- Excellent knowledge of resident care management principles and best practices.
- Exceptional leadership and organizational skills, with the ability to coordinate and oversee the work of a healthcare team.
- Outstanding communication and interpersonal skills, with the ability to establish and maintain effective relationships with residents, their families, and team members.
- Strong advocacy skills, with a commitment to promoting residents' health and wellbeing.
- Proficiency in using healthcare information systems and software.
- A current, valid license to practice as a Registered Nurse or equivalent.
- A commitment to ongoing professional development and learning.
