Job Summary:
BK Outlooks is seeking a skilled Physical Therapist to provide home health services to patients in the Chicago area. As a Physical Therapist, you will assess patient functioning levels, develop treatment programs, and provide education to patients and caregivers.

Responsibilities:

  • Conduct thorough assessments of patients' physical and cognitive functioning levels
  • Develop and implement individualized treatment programs to improve patients' independence and functioning
  • Provide education and training to patients and caregivers on therapy programs and device use
  • Record and report patients' progress and reaction to treatment
  • Supervise physical therapy assistants and certified nursing assistants as needed
  • Prepare reports and participate in staff development activities

Requirements:

  • Active PT state license (IL state)
  • Master's degree
  • 1 year of experience in a healthcare setting
  • Valid driver's license and ability to travel to patients' homes
