Job Summary:The Physical Therapist will be responsible for evaluating, planning, and implementing physical therapy services for assigned patients. This includes assessing patients' needs, developing and implementing individualized treatment plans, and evaluating treatment outcomes.
Responsibilities:- Evaluate patients' physical abilities and develop treatment plans to improve mobility, strength, and function.
- Implement physical therapy interventions, including exercises, modalities, and other treatments, to help patients achieve their rehabilitation goals.
- Document patient progress and outcomes, and communicate with other healthcare professionals to ensure comprehensive care.
- Participate in interdisciplinary team meetings to discuss patient care and develop treatment plans.
- Assist with transporting patients to and from physical therapy sessions, as needed.
- Current California Physical Therapist license.
- Minimum two years' experience as a Physical Therapist in a nursing, hospital, or other related medical facility.
- Training in rehabilitative and restorative therapy practices.
- Current CPR/BLS certifications.
- Proven organizational and communication skills.
