Job SummaryWe are seeking an experienced On-Call Registered Nurse to join our Hospice Team in Akron. As a key member of our team, you will be responsible for providing compassionate care to patients and their families in their homes, skilled nursing facilities, or residential care facilities.
Key Responsibilities- Respond to calls from patients, families, caregivers, and referral sources in a timely and professional manner.
- Assess patient needs and develop individualized care plans to support their physical, emotional, and spiritual well-being.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Document all patient interactions, interventions, and care plan changes in accordance with company policies and procedures.
- Provide education and support to patients and their families on end-of-life care, pain management, and other relevant topics.
- Participate in on-call rotations to provide nursing services to clients outside of regular office hours.
- Graduate of an accredited school of nursing.
- Valid license as a Registered Nurse and current Basic Life Support certification.
- Minimum one year of recent professional nursing experience, preferably in hospice or home health care.
- Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
- Computer skills, including Microsoft Word and email, are required.
