Main Responsibilities:

  • Clinical Expertise: Deliver high-quality primary care services to a panel of members, including ISNP Model of Care activities, advanced care planning, and treatment in plan models to avoid unnecessary hospitalizations.
  • Care Team Collaboration: Facilitate communication and coordination with members' PCPs, loved ones, and interdisciplinary care team members to ensure seamless care delivery.
  • Nursing Home Environment: Establish meaningful relationships with nursing home staff and colleagues, ensuring effective functioning within the facility environment.
  • Member-Centric Approach: Focus on delivering exceptional customer service, prioritizing members' needs, and ensuring high-quality care outcomes.
  • Professional Development: Engage in ongoing education and professional growth to enhance clinical skills, knowledge, and best practices.

Requirements:

  • Qualifications: Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program or equivalent experience; National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner.
  • Credentials: State unrestricted licensed Advanced Practice Nurse, DEA License, active national provider number (NPI), and Medicare provider number.
  • Experience: 6+ months experience as a Nurse Practitioner, preferably in a nursing home environment.
