Job Summary

YAI is seeking a Therapeutic Coach to join our team. The successful candidate will provide supportive services to individuals with intellectual and developmental disabilities, focusing on behavioral health and therapeutic interventions.

Key Responsibilities:

  • Implement person-centered therapeutic interventions in individual homes and support primary caregivers in implementing these interventions.
  • Conduct observational assessments and provide feedback to the START team on biopsychosocial factors influencing current functioning, effects of interventions, and coaching outcomes.
  • Offer consultation and hands-on education to entire family/support systems to create a safe, nurturing, and meaningful home environment.
  • Assist individuals with daily activities, personal care, behavioral health needs, and health, wellness, and leisure activities.
  • Maintain professional boundaries with individuals and families receiving services and follow ethical and licensing guidelines.
  • Participate in development and implementation of persons' cross-system crisis prevention and intervention plans.
  • Plan, structure, and prompt social and leisure time activities, including activities that build skills for positive supports, stress reduction, health and wellness, communication, adaptive living, and daily living.
  • Serve as a positive representative for the START Program with other professionals, families, and the community.
  • Maintain timely documentation, progress notes, and other data, including incident and accident reports.
  • Respond to individual safety needs, including reporting significant changes in behavior and health.

Requirements:

  • Bachelor's degree in psychology, social work, or other human service-related field.
  • Minimum of 2 years' experience working with individuals with IDD and behavioral health needs.
  • Preferred qualifications include I/DD experience, nursing homes, rehabs, or chronic/geriatric care experience.

Compensation: $16.90 per hour.
