Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in the Kettering/Centerville area. As an LPN, you will be responsible for assessing, planning, implementing, and evaluating the nursing care for a caseload of clients. You will work closely with our interdisciplinary team to coordinate care and ensure the best possible outcomes for our clients.

Key Responsibilities:

  • Assess clients' needs and develop individualized care plans
  • Coordinate and supervise the delivery of nursing care by home health aides
  • Initiate and complete necessary documentation and reporting
  • Communicate with physicians and other healthcare professionals
  • Maintain a safe and healthy environment for clients and staff

Requirements:

  • Valid driver's license and safe driving record
  • Licensed Practical Nurse from an accredited school of nursing with a current Ohio license
  • Two years of nursing experience, with one year in home health care preferred
