Job Opportunity

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Mymedtrust. This is an exciting opportunity to work in a dynamic environment and make a meaningful contribution to the care of patients with traumatic brain injury.

Key Responsibilities
  • Participate in all phases of care coordination, ensuring seamless transitions between healthcare providers.
  • Provide nursing expertise to coordinate care processes, including assessment, planning, implementation, coordination, and monitoring.
  • Develop and implement local strategies to support case management, including inpatient, outpatient, onsite, and telephonic follow-ups.
  • Design and implement tools to facilitate case management, such as patient identification and assessment, clinical practice guidelines, algorithms, and databases for community resources.
  • Integrate coordination of care and utilization management with nursing case management and social work case management.
Requirements
  • Possess an Associate degree from an accredited nursing program.
  • Have a minimum of 1 year of experience in case management.
  • Hold a valid Registered Nursing license from any US jurisdiction.
  • Be certified in Basic Life Support from an American Heart Association Program or the American Red Cross.
  • Have experience in TBI-related casework.
