Key Responsibilities- Admissions and Social Services Evaluations: Complete thorough assessments of guests' needs, providing recommendations for social services or referrals as needed.
- Financial and Legal Support: Assist guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and preplanning arrangements with funeral homes.
- Discharge Planning: Coordinate with guests, families, and outside agencies to ensure safe discharges and transitions to community congregate living, home care services, or other facilities.
- Counseling and Psychiatric Services: Provide or arrange needed counseling services and psychiatric medication management as indicated.
- Documentation and Record-Keeping: Ensure accurate and timely documentation in MDS care plans.
- Current Social Work Licensure: Possess current licensure in the state.
- Education and Experience: Bachelor's degree in social work required; master's degree preferred. One year of supervised social work experience in a healthcare setting is preferred.
