LPN Navigator

At Katy Trail Community Health, we're seeking a skilled LPN Navigator to join our team in Sedalia. As a Full-Time PrEP LPN/Navigator, you'll play a vital role in empowering individuals through education and support in PrEP services.

Your day as a PrEP LPN/Navigator will be both varied and rewarding. You'll primarily be responsible for conducting assessments, educating patients on PrEP, and providing emotional support throughout their healthcare experience.

• Conduct assessments and educate patients on PrEP
• Provide emotional support and navigate patients through their healthcare journey
• Collaborate closely with healthcare providers to ensure seamless patient navigation and care coordination

• Strong interpersonal and communication skills
• Demonstrated empathy and understanding while building rapport with patients from diverse backgrounds
• Active listening skills to effectively address patient concerns and provide tailored education about PrEP
• Organizational skills to manage patient records and coordinate care efficiently
