Overview:
As a Corporate Care Liaison, you will play a pivotal role in collaborating with designated hospitals and referral networks under the guidance of the Chief Executive Officer (CEO) and the Corporate Care Liaison Team Leader.
Key Responsibilities:
1. Conduct comprehensive patient evaluations and facilitate seamless transitions of care into ABCM Corporation's facilities.
2. Perform preadmission and readmission assessments, ensuring compliance with regulatory standards for continuity of care by obtaining necessary medical documentation.
3. Engage with prospective admissions and their families to provide detailed information about care center offerings, clarify services, assist in financial inquiries, and address any questions.
4. Maintain consistent communication with Iowa acute care health centers and ABCM care centers regarding anticipated discharges to skilled nursing facilities or long-term care.
5. Collaborate with nursing staff at care centers to identify potential resident needs and ensure that required supplies and equipment are prepared for incoming admissions.
6. Foster relationships, educate, and promote services to referral sources to enhance collaboration and support.
Qualifications:
- An active and unrestricted Iowa (or compact state) RN or LPN license, or a Bachelor’s degree in Social Work or Human Services (relevant work experience may substitute for formal education).
- Preferred experience in long-term care (LTC) and/or acute care settings, with a solid understanding of Medicare and Medicaid regulations.
- Must demonstrate compassion, attention to detail, and possess exceptional communication and interpersonal skills.
- Flexibility to work varied hours, including weekends and on-call duties, is essential.
- Reliable transportation for travel is required.
Compensation and Benefits:
- Annual Salary Range: $60,000 - $70,000, negotiable based on experience.
