Cedars-Sinai is seeking a skilled Nurse Navigator to join our Congenital Heart Program. As a key member of our team, you will play a vital role in providing exceptional patient care and support to families affected by congenital heart disease.

• Develop and implement individualized care plans for patients and families, ensuring seamless coordination of care across the continuum.
• Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to provide comprehensive care and support.
• Provide education and guidance to patients and families on congenital heart disease, treatment options, and post-delivery care.
• Track and analyze clinical outcomes, identifying areas for quality improvement and implementing evidence-based practices.
• Lead quality improvement initiatives and projects, ensuring alignment with Cedars-Sinai's commitment to excellence and patient-centered care.

• Bachelor's degree in nursing required; Master's degree preferred.
• Current, unrestricted California RN license required.
• Minimum 3 years of experience in a pediatric setting, with a focus on congenital heart disease and cardiology preferred.
• Ambulatory Nurse Certification or other specialty nursing certification required (or willingness to obtain within one year from start date).
