Overview:

As a case manager, you will play a vital role in developing comprehensive care plans in collaboration with patients. Your responsibilities will include monitoring ongoing health conditions, ensuring continuity of care, analyzing patient claims data, reviewing laboratory results and prescriptions, and empowering patients to take charge of their health while collaborating with healthcare providers to establish treatment goals.

Key Responsibilities:

  • Conduct disease management for patients with chronic conditions.
  • Perform initial assessments to evaluate patients' understanding of their health issues and identify educational needs.
  • Collaborate with patients to formulate disease-specific management plans tailored to their individual goals.
  • Monitor patient progress by tracking attendance at appointments and evaluating treatment responses.
  • Advocate for necessary services and resources, providing support during crises.
  • Offer ongoing education and assistance to patients and their families, either in-person or via phone, until they transition back to their primary care provider.
  • Ensure timely follow-up for patients discharged from inpatient care within 7 days.
  • Communicate care plans effectively with physicians, patients, and the healthcare team.
  • Continuously assess patient needs and adapt care plans accordingly.
  • Maintain accurate patient records by documenting case notes and progress.
  • Ensure timely outreach to members enrolled in the Disease Management program.

Education and Certification:

  • Completion of an accredited Practical or Vocational Nursing program (Required)
  • BLS Certification from American Heart Association or equivalent
  • Licensed Practical Nurse (LPN) license in the State of Florida

Additional Information:

This role requires critical thinking, sound judgment, and the ability to work independently in a fast-paced environment. A minimum of one year of LPN experience is required. Employment is contingent upon successful completion of a Level II Criminal History record check.

Physical Requirements:

  • Frequent keyboard entry and patient interaction.
  • Ability to lift and carry patients weighing 35 pounds or more.
  • Requires prolonged periods of sitting and standing.
  • Exposure to various biological and environmental hazards.

Shift:

Standard office hours, not eligible for shift differential.
