Job Summary

We are seeking a skilled Case Manager to join our interdisciplinary team. The successful candidate will be responsible for coordinating patient care, developing treatment plans, and collaborating with healthcare professionals to achieve optimal patient outcomes.

Key Responsibilities

  • Complete departmental orientation and initial competencies
  • Consult with other departmental staff to collaborate in patient care delivery and identify barriers to care
  • Document patient information per workflow timeline and content requirements
  • Schedule family conferences and communicate with caregivers to keep patients informed of progress
  • Coordinate weekly patient care team conferences to develop and refine treatment plans
  • Review patient assigned CMG and identify potential missed comorbid conditions
  • Perform other duties as assigned

Requirements

  • Current Registered Nurse or Social Work licensure or Healthcare professional licensure
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum of 2 years social work or case management experience in an inpatient setting
  • Effective oral and written communication skills in English
  • Basic computer skills in excel, word, outlook, power point, etc.
