Practical Nurse

We are seeking an experienced LPN to join our team at Children's Home of Jefferson, where you will provide compassionate and skilled care to clients in need. As a member of our outpatient behavioral health team, you will work closely with a multidisciplinary treatment team to ensure comprehensive care and support.
Duties and Responsibilities
- Provide direct nursing care to clients, including vital signs monitoring and medication administration
- Develop and implement individualized treatment plans, in collaboration with the treatment team
- Conduct educational sessions on medication management and adherence
- Coordinate with pharmacies and other clinical staff to ensure seamless care delivery
- Document all interactions, interventions, and progress in client records
Skills and Qualifications
To succeed in this role, you must have a current NYS LPN license, prior experience in nursing, preferably in pediatrics or gerontology, and excellent communication and interpersonal skills. A valid NYS driver's license is required.
Work-Life Balance
