Hartford HealthCare is excited to present a new role for a Pulmonary/Sleep Medicine Nurse Practitioner or Physician Assistant dedicated to delivering outpatient care within Hartford County.

Responsibilities:

  • Assist Pulmonary and Sleep Medicine Physicians with patient referrals and outpatient management.
  • Deliver medical services under physician supervision.
  • Conduct new patient assessments, urgent care visits, follow-up appointments, and ongoing patient management.
  • Perform comprehensive physical examinations and patient interviews to ensure accurate diagnoses.
  • Order and interpret diagnostic tests to identify health conditions.
  • Prescribe medications as necessary.
  • Monitor patient progress and adjust treatment plans accordingly.
  • Maintain and update patient medical records.

Qualifications:

Applicants must be eligible for licensure as a Physician Assistant (PA) or Advanced Practice Registered Nurse (APRN) in Connecticut. Nurse Practitioners must be eligible for certification as a Family Nurse Practitioner (FNP) or Adult Gerontology Primary Care Nurse Practitioner (AGPCNP).
