We are seeking a skilled Occupational Therapist to join our team in providing high-quality home health services. As a member of our Resilient Healthcare team, you will play a vital role in helping patients regain independence and improve their overall quality of life.

About the Role:

  • Develop and implement effective occupational therapy plans to support patients in achieving their goals.
  • Evaluate patient needs and provide personalized interventions to enhance their daily living skills.
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication.
  • Conduct thorough assessments to identify areas for improvement and develop strategies to address them.
  • Provide education and training to patients and their families on proper techniques and equipment usage.

Responsibilities:

  1. Conduct initial evaluations to assess patient needs and develop individualized plans.
  2. Implement occupational therapy interventions, including exercises, activities, and training sessions.
  3. Maintain accurate records of patient progress and update treatment plans as needed.
  4. Communicate effectively with patients, families, and healthcare teams to ensure coordinated care.
  5. Stay up-to-date with industry developments and best practices to deliver evidence-based care.

Requirements:

  • Current licensure as an Occupational Therapist in the state of practice.
  • Master's degree or higher in Occupational Therapy from an accredited institution.
  • Proven experience in home health or related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively as part of a team.
